All students must sign this waiver of liability. By signing this waiver of liability, the student, parent, and/or guardian acknowledges that they have read this agreement and will abide by all the rules and codes of conduct of the Caio Terra Academy LLC & Caio Terra Academy Woodland. As well as agreeing to wear all mandatory safety equipment. Signing this waiver also means that the undersigned/student releases Caio Terra Woodland and all of the Owners, Bill McInerney, Coaches, Instructors, Contractors and Employees - as well as Caio Terra Academy LLC, Nick Haloski, Caio Terra, and all of the employees, contractors, or instructors of Caio Terra Academy LLC from any liability in case of injury up to and including death results from the training or instruction at Caio Terra Woodland and/or Caio Terra Academy LLC whether caused by another student, instructor or self.

The undersigned/student acknowledges that Brazilian Jiu Jitsu (BJJ) strength and conditioning, and all our programs are physical activities, and the participation may result in serious injury up to and including death or other damage, due not only to the undersigned/students actions, inaction or negligence but also to that of others associated with or present at these or other classes.

Furthermore, I acknowledge that there may be risk not known to me and are not reasonably foreseeable at this time. I personally, totally, and completely assume all risk involved or in any way related to my participation in the programs offered whether known or unknown to me at the present time. Participation in any program at Caio Terra Academy Woodland and/or Caio Terra Academy LLC is voluntary and undertaken entirely of my own free will. I hereby represent and certify that I am in good physical condition, and have no diseases, injuries, or other physical or mental conditions that would impair my ability to participate in any program offered. The undersigned/student, heirs, executors, and successors hereby waive ANY claim of accidental and or negligent tort damage’s or injury against Caio Terra Woodland, Caio Terra Academy LLC, it’s owners, coaches, instructors, contractors, employees Bill McInerney, Nick Haloski, Caio Terra, and any physical location where Caio Terra Woodland and Caio Terra Academy LLC classes are taught.
