All students must sign this waiver of liability. By signing this waiver of liability, the student, parent, and/or guardian acknowledges that they have read this agreement and will abide by all the rules and codes of conduct of CompoundX LLC & Woodland Jiu Jitsu Academy. As well as agreeing to wear all mandatory safety equipment. Signing this waiver also means that the undersigned/student releases Woodland Jiu-Jitsu Academy and all of the Owners, Bill McInerney, Coaches, Instructors, Contractors and Employees - as well as CompoundX LLC from any liability in case of injury up to and including death results from the training or instruction at Woodland Jiu-Jitsu Academy and/or CompoundX LLC whether caused by another student, instructor or self.
The undersigned/student acknowledges that Brazilian Jiu Jitsu (BJJ) strength and conditioning, and all our programs are physical activities, and the participation may result in serious injury up to and including death or other damage, due not only to the undersigned/students actions, inaction or negligence but also to that of others associated with or present at these or other classes.
Furthermore, I acknowledge that there may be risk not known to me and are not reasonably foreseeable at this time. I personally, totally, and completely assume all risk involved or in any way related to my participation in the programs offered whether known or unknown to me at the present time. Participation in any program at Woodland Jiu-Jitsu Academy and/or CompoundX LLC is voluntary and undertaken entirely of my own free will. I hereby represent and certify that I am in good physical condition, and have no diseases, injuries, or other physical or mental conditions that would impair my ability to participate in any program offered. The undersigned/student, heirs, executors, and successors hereby waive ANY claim of accidental and or negligent tort damage’s or injury against Woodland Jiu-Jitsu Academy, CompoundX LLC, it’s owners, coaches, instructors, contractors, employees Bill McInerney, and any physical location where Woodland Jiu-Jitsu Academy and CompundX LLC classes are taught.
